When staff share a concern, we respond with appreciation. Saying, “I’m glad you mentioned it,” reinforces trust and keeps communication open. This approach encourages real-time reporting and strengthens teamwork.
Stand-up meetings and rounding anchor early detection in our daily workflow. During stand-up, staff share observations and discuss next steps. Consistency makes it easier to recognize when something falls outside a resident’s normal pattern.
Within the Wellness team, shift exchange gives every team member a voice. Care Partners and Medication Partners share insights alongside Nurse Managers. This collaboration ensures we consider every perspective and act quickly when needed.
